For Your First Visit

Your initial evaluation should take approximately an hour.  Please arrive about 15 minutes early for your first treatment to complete the necessary paperwork.  Filling out all your paperwork prior to your first appointment will speed up the check-in process.

On your first visit to Physical Therapy, please be sure to bring:

1.  Your driver's license or identification card
2.  Your prescription for Physical Therapy
3.  Your insurance card
4.  Comfortable clothing

It is the responsibility of each patient to verify his or her Physical Therapy benefits with their insurance companies prior to initiating care.  As a patient courtesy, we will do our best to confirm coverage with each patient's insurance plan, but that information is not a guarantee of benefits.
